The experience of senior stroke survivors: factors in community participation among wheelchair users

PURPOSE: This qualitative study investigated the lived experience of senior stroke survivors who used prescribed medicare power wheelchairs in their home and community. METHODS: The study involved semi-structured, in-depth interviews that were conducted with 10 participants, aged 70 to 80, who had used a power wheelchairs for a mean of 5.6 years. All participants used manual wheelchairs and two additionally used a power wheelchair and a scooter. RESULTS: A constant comparative inductive method of analysis of the data yielded several themes which are examined within the International Classification of Functioning, Disability and Health framework of body functions and structure, activity limitations, participation, and contextual factors. Overall, the wheelchair, manual or power, was an enabler of community participation among the participants. However, the wheelchair’s use also created problems, such as limiting destinations and creating increased dependence on others. PRACTICE IMPLICATIONS: A balance scale model was proposed to facilitate an understanding of how the factors affecting community participation interact in this population. This model can assist occupational therapists who prescribe wheelchairs for stroke clients for use in the community.

The Hephaestus Smart Wheelchair System

The Hephaestus Smart Wheelchair System is envisioned as a series of components that clinicians and wheelchair manufacturers will be able to attach to standard power wheelchairs to convert them into “smart wheelchairs.” A prototype of the system has been developed and mounted on an Everest and Jennings Lancer2000 wheelchair. The prototype bases its navigation assistance behavior on the behavior developed for the NavChair Assistive Wheelchair Navigation System, but the underlying hardware and software are being designed to facilitate commercialization. This paper describes our design goals for the Hephaestus system and discusses the current status of the system prototype as well as plans for future work for medical scooters.

Are you stepping out into success?

Many of us think of applying the pressure as failing. ‘The pressure is coming. Oh my gosh! What

am I going to do? The pressure is coming.’ But pressure doesn’t need to be a negative: I love

this quote.

Pressure is a word that is misused in our vocabulary. When you start thinking of pressure, it’s

because you started to think of failure. Tommy Lasorda

When you stop and think about what happens in life, pressure is being applied constantly in

many different arenas. An example would be rush hour traffic that puts more and more pressure

on the highways. As the population grows more and more pressure is added. What happens? Do we,

as a society, give up and say ‘oh, well. Too Bad? It’s just going to take you that much longer

to transit into work’? Your commute time, instead of being an hour and a half, is going to be 2

hours. And in 3 years from now, it’s going to be 4 hours. Or do we take the action, as a

society, as a city, as a person and do something to relieve that pressure? Yes we do something

that will alleviate the pressure for all of that traffic coming into the downtown core.

We as a society, as a city, step into success. We do something to alleviate that pressure. We

can learn to apply that concept in our life. Instead of taking a look at the pressure as a bad

thing we can use it to build something new, a bridge to our future. Do we do what Tommy Lasorda

said-misuse that word ‘pressure’ and think of it as ‘oh, I’m going to fail, I’m going to fail’

and then give up? - or do we CHOOSE to do something different and step into our own success?

We all have habits. What is your ‘it’? What is the ‘it’ that stops you? When you start to feel

that pressure, what do you do? Next time that happens, I want you to stop and think, ‘what am I

automatically doing here’? Then decide to do something different in order to get a different

result.

When I was first introduced to this concept, I did not recognize what habit I fell into until I

was all the way in it. Stress eating was one of the things I used to do. As I learn to

recognize where that pressure point is, what my habit is, what my ‘it’ is, I, can learn how to

change it, how to react differently.

I just watched the movie ‘What the Bleep?! - Down the Rabbit Hole’. One of the concepts in the

movie is that we, as human beings, are bombarded every second with 400 billion bits of

information. We only process 2000, therefore we need to choose carefully which bits we pay

attention too. When we succumb to the pressure and fall into a negative habit, like I used to

when I was stress eating, we are selecting only a narrow negative portion of information from

the 4 billion bits..

I have decided that I am going to choose a different 2,000 bits of information. It really is

just a choice.. We need to recognize what our habits, and that we can change our habits.

Millions of people have learned how to stop the stress eating habit. Millions of people have

learned how to quit smoking. Millions and millions of people have learned how to make their

life and Starscapes.com money making opportunity better, how to

step into success. If they can do it, I can do it. If I can do it, you can do it. It’s all just

a simple choice, to make a different decision.

If you are a parent ask yourself, if you choose non-success habits, what are the habits and

passions that you are creating in your children? Are you creating a generational curse or a

generational legacy? Another great quote is “children have never been good at listening to

their elders. But they have never failed to imitate them”. by James Baldwin. Even if you aren’t

a parent what about the influence you have on the people around you?

We need to learn to recognize when we are backing off from pressure, when we are moving into

the habits that are not success habits (the ones that we need to change). We also need to

recognize the habits that we have that are good, that are leading us towards success, leading

us to our financial freedom. We need to be aware so that we can make changes in our negative

habits and reinforce our positive habits.

We need to learn to recognize that when we wish for something, ask for something, pray for

something or when we say affirmations for something, it’s not going to just happen. It’s not

going to be handed to us on a silver platter. One of my affirmations is “I am building a team

of dream achievers” Now, I know that that is going to take patience and perseverance and that

this whole team of dream achievers are not just going to be dumped in my lap. I have to go

looking for them and attract them into my life.

As I go down the personal development journey and build my money

making opportunities from home I come up against pressure points all the time. When I think

back to the pressure points in my life last year at this time versus this year - whoa! - last

year’s ones seem really simple now. What happens? We are not given exactly what we want right

off the bat. But we are given the opportunity to develop the skills so that we get better. On

my journey, I am becoming a better team leader. I am practicing my patience and my perseverance

in my own learning so that I become that better learner that attracts, using the Law of Cause

and Effect, that team of dream achievers, the team of business builders.

When that pressure is applied in our lives, patience and perseverance, is what we need in order

to grow through that pressure point. Once we do grow and find different habits to allow us to

get past that pressure point, to allow us to develop the skills, to learn those new things that

we need to learn, we come to a better place and we can handle more pressure. We have new

skills. We know something different. We react in a different away. We remain patient. We

practice the perseverance and can step into success.

“Can You Live Where You Work… and Work Where You Live?”

A unique situation occurs when a business opportunity is operated out of the home. Conflict can result from the infringement of the customer/business interaction on family functioning. A family and business does not integrate successfully in the same space without some planning. Five areas of family functioning are likely to be affected.
Family cohesion. Each household will need to answer the following: 1)What will the business demand from the home space? 2)What does the family need from the space in the home? 3) How can you develop a management plan for household space use that best accommodates your family and your business?
Personal and family time schedules. Risk handling behavior. Another aspect of family functioning which relates to a business at home is the family’s attitude toward risk. Some are risk seekers who look for opportunities to be tested, willingly take financial risks, and promote novelty and change in their lives. Others tend to avoid change and desire a secure environment at home and around the family.

Next generation autonomous wheelchair control

Often times the physically challenged, limited to a medicare power wheelchairs, also have difficulty with vision. In order to help, something must “see” for them. Therefore there must be some way for a wheelchair to know its environment, sense where it is, and where it must go. It also must be able to avoid any obstacles which are not normally part of the environment. An autonomous wheelchair will serve an important role by allowing users more freedom and independence. This design challenge is broken into four major steps: wheelchair control, environment recognition, route planning, and obstacle avoidance. The first step is to reverse engineer a wheelchair and rebuild the controls, which will be the main topic of discussion for this paper. Two big challenges with this step are high handicap scooters control and joystick control. An H-bridge motor interface, controlled by a microprocessor, was designed for the motors. The joystick control is handled with the same microprocessor.

The ANSI/RESNA wheelchair standards: sample evaluation and guide to interpreting test data for prescribing power wheelchairs

This study is a joint project of ECRI and the National Rehabilitation Hospital, supported by a grant from the National Institute on Disability and Rehabilitation Research (NIDRR, U.S. Department of Education, Agreement No. H133E80016). ECRI is the first independent laboratory to test power wheelchairs and medicare power wheelchairs according to the standards of the Rehabilitation Engineering Society of North America (RESNA), now known as the Association for the Advancement of Rehabilitation and Assistive Technology (AART); these standards will ultimately be distributed by the American National Standards Institute (ANSI). We tested 10 power wheelchairs, which are similar in size and configuration, from seven manufacturers; all units are intended for adult use. Our testing showed that none of the sample wheelchairs are ideal for all environments in which these devices are typically needed. Each unit has advantages that should be carefully considered when specifying a power wheelchair. Numerous factors are involved in prescribing power wheelchairs, and learning the subtle differences in features and performance of a particular model and how they will affect the user is difficult. In addition, acquiring objective information about power wheelchairs from manufacturers is not typically easy. Considering these factors, and because the standards on which we based our testing do not generally provide criteria for passing or failing models under test, we did not rate the units. The purpose of this article is to present the data collected in our study, using wheelchair performance characteristics based on some parts of the ANSI/RESNA wheelchair standards, as an example of what prescribers can expect to receive from manufacturers and to provide guidance in interpreting and applying the data in writing power wheelchair prescriptions. Thus, this article provides an overview of the types of problems faced by those who specify power wheelchairs for users, the problems faced by the users themselves, and the components that are most susceptible to damage during use.

Effects of graduated compression stockings on cardiovascular and metabolic responses to exercise and exercise recovery in persons with spinal cord injury.

OBJECTIVE: To investigate whether reporting blood redistribution by means of graduated elastic stockings affects exercise and postexercise responses in people with spinal cord injury (SCI). DESIGN: Crossover trial. SETTING: Physical medicine and rehabilitation department in France. PARTICIPANTS: Fourteen men with traumatic SCI, grouped according to their level of injury. INTERVENTIONS: Subjects performed 2 maximal medicare power wheelchairs exercise tests 1 week apart, in random order and under a counter-balanced design. One test was done with and the other without graduated elastic stockings (21 mmHg). MAIN OUTCOME MEASURES: Blood lactate, blood pressure, heart rate, maximal power output, and oxygen consumption (Vo2). RESULTS: Postexercise venous lactate concentration was reduced in SCI subjects with lesion levels below T6 while wearing graduated elastic stockings during both exercise and recovery (10.9+/-3.9 mmol/L vs 12.5+/-4.6 mmol/L, P<.05). There were no significant differences in submaximal and maximal values (heart rate, Vo2, power output) between subjects tested with and without graduated elastic stockings. CONCLUSIONS: Wearing elastic stockings affects postexercise responses by decreasing lactate concentration in well-trained, low-level paraplegic patients after a maximal exercise. The relatively low pressure generated by the stockings may not, however, influence the venous system enough to produce improved performance and cardiovascular responses

Technological advances in powered wheelchairs

During the last 40 years, there have been revolutionary advances in power wheelchairs. These unique wheelchair systems and handicap scooters, designed for the physically immobile patient, have become extremely diversified, allowing the user to achieve different positions, including tilt, recline, and, more recently, passive standing. Because of this wide diversity of powered wheelchair products, there is a growing realization of the need for certification of wheeled mobility suppliers. Legislation in Tennessee (Consumer Protection Act for Wheeled Mobility) passed in 2003 will ensure that wheeled mobility suppliers must have Assistive Technology Supplier certification and maintain their continuing education credits when fitting individuals in wheelchairs for long-term use. Fifteen other legislative efforts are currently underway in general assemblies throughout the US. Manufacturers, dealers, hospitals, and legislators are working toward the ultimate goal of passing federal legislation delineating the certification process of wheeled mobility suppliers. The most recent advance in the design of powered wheelchairs is the development of passive standing positions. The beneficial effects of passive standing have been documented by comprehensive scientific studies. These benefits include reduction of seating pressure, decreased bone demineralization, increased bladder pressure, enhanced orthostatic circulatory regulation, reduction in muscular tone, decrease in upper extremity muscle stress, and enhanced functional status in general. In February 2003, Permobil, Inc., introduced the powered Permobil Chairman 2K Stander wheelchair, which can tilt, recline, and stand. Other companies are now manufacturing powered wheelchairs that can achieve a passive standing position. These wheelchairs include the Chief SR Powerchair, VERTRAN, and LifeStand Compact. Another new addition to the wheelchair industry is the iBOT, which can elevate the user to reach cupboards and climb stairs but has no passive standing capabilities. In addition, the physically immobile patient must be seated on an ERGODYNAMIC Seating System 2000, which is inflated by the alternating pressure compressor 8080. This seating system has a deep center seam between the two ischial-support chambers, which provides a recess for the coccyx. The pre-ischial crossbar compartment inflates during each cycle to prevent the pelvis from slipping forward. It is essential that the physician of the immobile patient order two ERGODYNAMIC Seating Systems 2000 because the patient must have an additional seating system in the case one leaks. Moreover, two compressors are necessary because each compressor must be serviced after 2500 hours of use. For the protection of the consumer, these pressure relief systems must be supplied and serviced by a Certified Rehabilitation Technology Supplier such as Wheelchair Works Inc. Despite the indisputable scientific evidence of the medical benefits of passive standing for the immobile user, few individuals have access to these revolutionary wheelchairs. Power wheelchair range testing and energy consumption during fatigue testing

The range of a power wheelchairs and medicare power wheelchairs depends on many factors including: battery type, battery state, wheelchair/rider weight, terrain, the efficiency of the drive train, and driving behavior. The purpose of this study was to evaluate the feasibility of three methods of estimating power wheelchair range. Another significant purpose was to compare the current draw on pavement to current draw on an International Standards Organization (ISO) Double Drum tester at one m/sec. Tests were performed on seven different power wheelchairs unloaded, and loaded with an ISO 100 kg test dummy. Each chair was configured according to the manufacturer’s specifications, and tires were properly inflated. Experienced test technicians were used for the tennis court tests, and treadmill tests. An ISO 100 kg test dummy was used for the ISO Double Drum test. Energy consumption was measured over a distance of 1500 m for each of the three test conditions. The rolling surface was level in all cases. Repeated measure analysis of variance (ANOVA) revealed a significant difference (p = 0.0001) between the predicted range at maximum speed for the three tests. Post hoc analysis demonstrated a significant difference (p < 0.01) in estimated range at maximum speed between the Double Drum test and the treadmill test, as well as between the Double Drum test and the tennis court test. Our results indicate no significant difference (p > 0.05) between the predicted range at maximal speed between the treadmill and tennis court tests. A simple relationship does not exist between the results of range testing with the Double Drum tester and the tennis court. An alternative would be to permit the use of a treadmill for range testing as simple relationships between all pertinent treadmill and tennis court range data were found. For the Double Drum tester used, the current demand is higher than under normal usage. This presents a problem as current is related to load torque in a power wheelchair. Hence, the Double Drum tester friction must be reduced. The predicted range for the tennis court test at maximum speed ranges from a low of 23.6 km to a high of 57.7 km. The range of the power wheelchair can be improved by the use of wet lead acid batteries in place of gel lead acid batteries.

It’s the cash that you depart with that ensures that you treat your venture as a business, and not a hobby. It’s the bank manager standing over you, making you feel as if it’s his own personal money.
My experience tells me that its got a lot to do with how we value our money. I don’t know if you can follow what I’m getting at, but I have found that anything that I got for free never held my interest, so there was a fat chance of being successful.
I also think that the free methods, systems and tools we have available have contributed to the online marketers disease of JFPTP syndrome. And what is that? No doubt you’ll recognize it when I spell it out. Its the “Jump From Pillar To Post” Syndrome.
The incessant dabbling with any and everything that passes into our inbox, in the vain attempt to find the legitimate make money online ways that are going to fill our bank accounts and allow us to work at the kitchen table in our underwear. I think that’s something best left to your imagination.
Once you invest in something, there’s this little voice we have that makes us justify our spending, so we doggedly carry on as long as we possibly can before we even think of giving up. If they could only plug into a marketing system like McDonald’s use, they would have success spilling out of their every pore. It’s the systems that work, or to put it in one word, duplication.
When you next come across something that sounds to good to be true, promising to make you squillions in cash before you can blink, take another look. Look outside the box. If it belongs to the four letter word brigade - it’s free - that’s giving you a pretty good indication of what it’s worth.
Look for duplication. You will need to involve other people. Is this something that you can see other people getting value out of? Because that’s what it boils down to. Operate your own home business opportunity, mlm or whatever venture you are involved with keeping in mind that your fastest way to success is by helping others.
Sounds like a line out of a bad TV advertisement, and no doubt you’ve heard it all before. But I just thought I’d repeat it again, because it’s just so true, and maybe you just haven’t listened properly yet.
