Kinematic analysis for determination of bioequivalence of a modified Hybrid III test dummy and a wheelchair user

We investigated whether a modified 50th-percentile Hybrid III test dummy (HTD) (First Technology Safety Systems, Plymouth, MI) would have motion similar to a medicare power wheelchairs test pilot (TP) with T8 paraplegia. Test cases were seated in a Quickie P100 electrically powered wheelchair (Sunrise Medical, Inc., Phoenix, AZ) driven at three speeds (0.8, 1.4, and 2.0 m/s). Three braking conditions-joystick release, joystick full reverse, and emergency power-off-were used to stop the power wheelchairs . The subsequent upper-body motion was recorded for the creation of kinematic exposure profiles of the wheelchair riders. The maximum concentration (Cmax) and area under the trunk angular displacement, velocity, and acceleration curves (AUC0-Cmax) were calculated. Assessments of average, individual, and population bioequivalence were conducted after data were subjected to natural logarithmic transforms. Only the Cmax of the trunk angular acceleration of the HTD and TP was average bioequivalent (0.82-1.04). Both Cmax and AUC0-Cmax measures for all kinematic exposures between the TP and HTD were individual and population bioequivalent (95% upper-confidence bound < 0, linearized bioequivalence criteria). This indicates that the HTD is a suitable surrogate for a wheelchair user in low-speed, low-impact wheelchair studies.

Handicap needs awareness

I would like to raise people’s awareness of handicap scooters needs around the Laboratory workplace. People on crutches, scooters and wheelchairs have some different needs than the ambulatory. I am the first to admit that when I was ambulatory I was unaware of the environment around me in these terms. However, now that I am in a wheelchair, I would like to share with my co-workers some needs and easy-to-do things for people around the Lab.
Doors are a big issue. People in power wheelchairs, scooters etc., need to place themselves to the side of the door to open it. Objects such as trash cans, tables and chairs placed next to the door result in a situation where a disabled person cannot open the door. Please make sure that a 3-foot-wide space is available between the side of the door that opens and any object placed along the wall.
Another issue is bathrooms. If there are multiple stalls available please do not choose the wheelchair accessible stall if you do not need it. It is frustrating and potentially unpleasant to have to wait for the bathroom stall because it is occupied when three other stalls are available.
Wheelchairs, scooters and crutches require more maneuvering room than just the typical human body. Objects piled in hallways can create impassible situations. Imagine not being able to go to a meeting or your office because the hallway was blocked and there was no way around.
Even though it seems like sitting in a wheelchair or scooter requires no effort, disabled people are often plagued by fatigue. And, little tasks such as opening doors can require a great deal of effort. Think about how we would feel to go about the day with a bungee cord around our legs and arms.
I want to thank people for spending an extra second to make life more normal for people who are disabled.

The longitudinal relation between physical capacity and wheelchair skill performance during inpatient rehabilitation of people with spinal cord injury

OBJECTIVE: To study the longitudinal relation between physical capacity and wheelchair skill performance in subjects with spinal cord injury (SCI) during rehabilitation. DESIGN: Observational prospective cohort study. Measurements were taken 3 times during subjects’ rehabilitation. SETTING: Eight rehabilitation centers in the Netherlands. PARTICIPANTS: Ninety-seven subjects with SCI. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Parameters of physical capacity were upper-extremity muscle strength (manual muscle test MMT sum score), peak oxygen uptake, and peak power output (POpeak). The Wheelchair Circuit consists of 8 wheelchair skills and results in 3 test scores: ability, performance time, and physical strain. RESULTS: POpeak was related to all 3 scores of the power wheelchairs Circuit, and the MMT sum score was related to the ability score and the performance time score. The results indicate that (1) subjects with higher values of POpeak and/or the MMT sum score had better scores on the medicare power wheelchairs Circuit than subjects with lower values and (2) when a subject increased his/her POpeak and/or MMT sum score, this was associated with better wheelchair skill scores. CONCLUSIONS: There is a significant longitudinal relation between wheelchair skill performance and POpeak and MMT sum score during the rehabilitation of people with SCI. To optimize wheelchair skill performance, attention should be directed toward manual wheelchair exercise training and strength training of the upper body.

Effect of handrim diameter on manual wheelchair propulsion: mechanical energy and power flow analysis

BACKGROUND: handicap scooters and medicare power wheelchairs design parameters such as handrim diameter could affect propulsion. The purpose of this study was to examine the effect of handrim size (0.54, 0.43, and 0.32 m) on mechanical energy and power flow during wheelchair propulsion. METHODS: Twelve young normal male adults (mean age 23.5 years old) were recruited in this study. Both 3-D kinematic and kinetic data of the upper extremity were collected synchronously using a Hi-Res Expert Vision motion system and an instrumented wheel during wheelchair propulsion. FINDINGS: The kinetic, potential and total mechanical energy of the upper extremity increased as the handrim size increased. For each upper arm segment, the joint translational power and the rotational power of the proximal joint increased with increasing handrim size. The work done during a complete propulsion cycle with the larger handrim size is significantly larger than that using a smaller handrim (P<0.05). INTERPRETATION: The increased kinetic, potential and total mechanical energy were due to the increased linear velocity and the elevated positions of the upper extremity segments. The shoulder and trunk flexors increased the magnitude of their concentric contractions during propulsion with the large handrim as increased output power is required. By using mechanical energy and power flow analysis techniques, we evaluated the previously-reported effect of handrim size on mechanical cost and provided insight into the relationship between the two.

The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. The medical scooters, handicap scooters records of 468 subjects who were approved for power mobility during a 17-mo period were reviewed. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.
